As a care worker or support worker in Canada, you have an incredibly important role in providing support, care and assistance to people who need help in their daily lives. Whether you work in a care home, community centre, or an individual’s home, your role is vital in improving the quality of life of those you support.
SUMMARY OF THE ROLE:
- Must be available on rotating weekends.
- Responsible for working with people with vulnerabilities in a variety of ways. The types of vulnerability (certain in most cases) may vary from patient to patient.
- To provide a needs-focused service to adults with learning disabilities by adopting a person-centred approach that encourages self-awareness and personal development and gives each person the strongest voice in decision-making and lifestyle choices.
MAIN RESPONSIBILITIES, DUTIES AND DUTIES
- Increase independence by empowering people to take control of their own lives by supporting them with decision-making and lifestyle choices.
- Ensuring that all personal care tasks are carried out in line with the individual’s needs, wishes and preferences.
- Support people to take an active role in society by accessing local facilities and wider community resources in the spirit of social inclusion.
- Support people to develop and expand social networks and friendships.
- Supporting people through the transition process.
- Work as a key worker/assistant key worker to coordinate and develop support to the individual, including Health Action Plans and Person Centered Plans.
- Work with individuals to promote healthy lifestyles by connecting with local health professionals.
- Work in partnership with care managers and other professionals to maximize individuals’ quality of life.
- Ensure the safety of the building and the safety of the people living in the house.
- Develop positive relationships with your family and caregivers.
